Miller is a man whose life has been shaped by war. A career soldier, he was an officer for the Spetsnaz GRU before the nuclear exchange in 2013 that obliterated the surface of the Earth with radiation. He has always felt a strong devotion to protecting his country, and this carried over into his life in the Metro where he faced the unbridled chaos of the adjusting survivors by forming a group of former soldiers and other devotees known as the Spartan Order. This group was created with the aim of maintaining a neutral space in the Metro which had quickly devolved into political and social conflicts, with a particular focus on protecting civilians and expanding the amount of resources available for survival. As the founder, Miller himself holds fast to these ideals while still indigent to his strong allegiance to the former Russian government. Although his outward personality is that of a hardened fighter, gruff and to the point, the respect shown to him by members of the Order comes largely from the unwavering loyalty he has for his own.
In Metro Exodus, Miller leaves the Metro after his son-in-law and fellow ranger, Artyom, discovers a secret of leading Metro faction Hansa: that those living underground in Moscow are not the only survivors of the war. Rather than turning him in, Miller abandons his position that he cares about greatly, allowing himself to be branded a traitor for the sake of protecting Artyom, his daughter, Anna, and his special operations squad. This is not unexpected, as Miller is always willing to make sacrifices for the good of those he cares about, Anna most of all. When she becomes ill from exposure to toxic gas while scouting, Miller blames himself for planting the seed of a NATO threat on the surface into her mind, and is prepared to do anything in his power to save her life. The relationship he has with Anna is strong, but somewhat shaky, as not long into her childhood, her mother committed suicide. She blames Miller because he was so laser focused on his work in the Order, and because the two of them were entrenched in a relationship where they enabled each others' alcoholism. After his wife's death, Miller has clung with a desperate tightness to his daughter, although his means of showing it is often overprotective rather than affectionate. While he gives her her freedom and even relies on her as one the best sharpshooters in the Order, he is overcome with worry whenever she appears to be in danger, and is especially harsh on Artyom when he feels it's his hopeful ideals that put her in harm's way. And while the underlying intention behind it may be good, he is a clear control freak in many ways, easily falling into habits that could be described as obsessive. This is most evident in his position as commander of the Order. He is so hellbent on keeping their operating station Polis and their group safe that he fixates on any threat that might come to disrupt what he's worked so hard to build. It isn't constructed solely from delusion by any means—Miller faced off against the Dark Ones with Artyom, and the Order was forced to mount a defense against the Reds in the battle for D6 where he lost his legs. Any position of power in the Metro is historically tenuous. However, Miller is also persistently paranoid about occupying forces of enemies from the war twenty years ago due to inside knowledge he received from Hansa, which is considerably less realistic.
Miller's relationship with Artyom is a huge part of what allows him to grow beyond his unhealthy fixation. After their collaboration against the Dark Ones, Miller developed a deep respect for him which also came hand-in-hand with a lofty amount of expectations. He is repeatedly shown to view Artyom's idealism as irresponsible because of how it ends up endangering his own life and often those around him, but at the same time, he trusts him, often more than any of his other rangers. When Artyom's beliefs about the outside world are proven to be true, Miller apologizes to him sincerely, and this growing admiration for him eventually leads to the decision to give him an anti-radiation serum that saves his life over Miller's own in the canon "good ending" to Exodus. Letting go and allowing Artyom to take control of the Order and Anna is, in effect, setting himself free from his own demons.
To sum him up, Miller is a stern leader, often quick to anger and derision—but also a peacemaker, loyal to a fault, and still a man that can occasionally find the time to have a laugh with his crew despite all the hardship.

Miller is a former Colonel for the Russian Spetsnaz prior to the event that plunged Moscow into the Metro meaning he was already quite in touch with combat and talking heads. In the Metro, he became one of the few willing to brave the outside world for resources, otherwise known as a "Stalker" and founded a group of rangers known as the Spartan Order. He has extensive experience in combat with both humans and mutants and is skilled in the use of firearms, knives, and in hand to hand combat. Just staying alive is a major concern in the wasteland outside the Metro, so he's developed a great sense of meticulousness when it comes to protecting his team.
As far as his physiology, Miller is a normal human.
